HERKIMER James M. Kowalczik, 43, of Herkimer, has been identified by state police as the driver who perished on June 18 when his car collided with a tree.
According to authorities, Kowalczik was driving a 2020 Jeep Grand Cherokee when he pulled off the road and collided with a tree.
West German Street in the village of Herkimer was the scene of the collision at 3:27 p.m.
The car was traveling east on West German Street when it left the north shoulder of the road and collided with a tree, according to the preliminary inquiry.
The only person in the car was Kowalczik.
After being taken by ambulance to Wynn Hospital, he was declared dead.
The State Police Bureau of Criminal Investigation is still looking into what caused the crash.
